You may send a questionnaire to us by E-mail, U.S. mail, or fax.

How to send via E-mail

  1. Click and complete one of the questionnaires to the side.
  2. In Word, click "File" > "Save as" and save the completed questionnaire to your desktop.
  3. Send the questionnaire as an attachment via e-mail to studies@emoryclinicaltrials.com
